Job SummaryWe are seeking a skilled Senior Case Manager to join our team at Professional Case Management. As a key member of our clinical team, you will be responsible for providing direct care and case management to assigned clients.Key Responsibilities

• Provide direct care and case management to assigned clients, ensuring adherence to Agency policy and state-specific nurse practice act.
• Collaborate with regional directors and other personnel to identify and correct issues, and improve services.
• Plan, implement, and evaluate care provided, participating in client care conferences as needed.
• Serve as a local on-site clinical resource, providing support to ensure client's home care needs are met.
• Assist with staffing coordination, ensuring appropriateness of staffing and scheduling of personnel within scope of practice, competencies, client needs, and complexity of home care.Requirements
  • Graduate of a state-approved school of professional registered nursing.
  • BSN preferred.
  • Current, unrestricted RN license in the state(s) of practice.
  • Minimum of two (2) years nursing experience, including one (1) year in home care or a closely related field.
  • One (1) year of supervisory and/or case management experience preferred.
  • Current CPR certification.What We Offer
