We take pride in our creative and collaborative work environment and in delivering clinically excellent, trauma-informed, recovery-oriented services.What are we looking for in a Nurse Care Manager?The Nurse Care Manager coordinates and supports the provision of medication assisted treatment services for incarcerated clients within the Kittitas County Jail. This program is a collaborative effort to support individuals who are incarcerated and experiencing opioid dependency by providing treatment while in jail and coordinating their transition to outpatient treatment services upon release to avoid gaps in care and maintain positive momentum once in the community. Working in coordination with other mental health and substance use treatment providers at the jail, the Nurse Care Manager is responsible for collaborative treatment and discharge planning, intake and induction of new patients, administering medications based on prescriber orders, and providing patient education. This position is responsible for managing the initiation, maintenance, and post-release transition process for medication assisted treatment within the jail setting, including ensuring that each treatment provider is involved appropriately throughout the process. The Nurse Care Manager position requires an understanding of substance use disorders and their treatment, supports the evidence-based model of medication-assisted treatment, and understands the potential risks and thus need to follow exactly all protocols & testing to provide safe treatment. Excellent communication and care coordination skills are essential in this role.Nurse Care Manager duties may include:

  • completing intake nursing assessments and scheduling MAT inductions
  • providing patient education about MAT and naloxone administration
  • administering medications (including injections) and monitoring clients' symptoms & response to medication
  • utilizing electronic dispensing software for medication administration
  • monitoring dispensing supplies, completing daily inventory, and following protocol for receiving shipments
  • collaborating with Comprehensive teams, corrections department staff, and partnering treatment providers to coordinate client care and make appropriate referrals
  • assessing clients for safety and risk to self or others, and taking appropriate crisis intervention measures when indicated
  • completing all documentation in a respectful, client-centered mannerQualifications:
    • Associate's or bachelor's degree in nursing
    • Registered Nurse or Licensed Practical Nurse in Washington State; RN preferred
    • Previous experience with substance use disorder treatment preferred
    • Must have valid driver's license and vehicle liability insurance
