Job Description:
This position is eligible for paid shift differentials!
Employees working evening hours (3:00 p.m. to 11:30 p.m. shift)
will receive $1.55 per hour over their regular rate of pay, and
employees working night hours (11:00 p.m. to 7:30 a.m. shift) will
receive an additional two dollars and fifty cents ($2.50) per hour
over their regular rate of pay. All staff members at Yakima Valley
Memorial must be in compliance with our Vaccination Policy, which
includes the COVID-19 (SARS-CoV-2) vaccine, by their first day of
employment. ROLE SUMMARY The LPN provides individualized patient
care integrating age related, cultural, and religious/spiritual
needs. The care delivered reflects the philosophy, goals and
objectives of the hospital and nursing services. The LPN is
accountable for own practice while under the supervision of the RN.
ESSENTIAL RESPONSIBILITIES+ + Collects, reports, and documents
objective and subjective data regarding the health status of the
patient/client, including biological, psychological, social,
emotional, spiritual, and age-specific, determinates of health. +
Implements a plan of care which consistently involves the patient
and family, and which demonstrates respect for patient rights,
privacy and confidentiality, provides input into plan of care
carries out plan of care. + Performs nursing care within scope of
licensure such as injections, administering medication, and other
duties as ordered. + Recognizes changes in patient condition,
observes effectiveness of pain management plan, and reports to RN
in a timely manner. + Collaborates in the revision and
effectiveness of the plan of care to achieve desired results. +
Provides and documents education of patient/client, family/support
person, other members of healthcare team, and students, under the
direction of the RN. + Participates in community education
presentations. + Maintains knowledge of current trends and
volunteers to share/present new information to staff. +
Participates in committees and develops resources to enhance
patient care under the direction of RN. + Plays an active role in
encouraging and communicating community development internally and
externally. + Maintains competency on all computer systems, and
documents care appropriately. + Participates in hourly rounding on
the unit. QUALIFICATIONS AND REQUIREMENTS Education: Completion of
a LPN program required. Experience: Previous healthcare experience
preferred. Licenses/Certificates/Registration: Washington State
(DOH) Licensed Practical Nurse license required. American Heart
Association BLS required. PALS and ACLS required within one year of
