Job Description:
All staff members at Yakima Valley Memorial must be in

Actual pay rate will be calculated based on relevant years of
experience and education/certification requirements.
We are looking for an RN, Case Manager to join our Home Health
team. The Home Health Case Manager is responsible for coordinating
the patient's care starting with admission. On average this RN will
be responsible for 18-20 patients, scheduling visits and weekly
case conferences to discuss challenging cases or changed
conditions. A secure message system supports collaboration across
the multi-disciplinary team. The Home Health Case Manager works
closely with the Primary Care or attending Physician to move toward
the patient's stated goals. We provide a 3-month long
preceptor-guided training.
ESSENTIAL RESPONSIBILITIES include:
1. Performs comprehensive subjective and objective assessment of
the patient status that includes physical, psychosocial, and
environmental parameters in home and assisted living settings.
2. Ensures compliance with federal/state regulations governing home
health care services by following policy and procedure consistent
with agency standards.
3. Formulates individualized plan of care that incorporates
analysis of assessment data including the patient's self-stated
goals; directs and participates in delivery of professional
services to home care patients.
4. Provides skilled interventions aimed at achieving realistic
outcomes within a specified time period.
5. Modifies and updates plan of care to reflect progress toward
outcomes.
6. Assures thorough and accurate documentation of all clinical data
from admission through discharge, including, but not limited to,
OASIS, HCFA 485, physician's orders, skilled nursing visits and
changes in conditions.
7. Submits documentation timely and per agency policy.
8. Maintains productivity standards as established by Compass
Care.
QUALIFICATIONS AND REQUIREMENTS
Education: Associate's degree in Nursing required; BSN
preferred.
Experience: Previous acute care nursing experience preferred.
Licenses/Certificates/Registration: Washington State (DOH)
Registered Nurse licensure required. American Heart Association BLS
required. Valid driver's license and proof of automobile insurance
required.
Additional requirements: Cell phone for use during working hours;
car for travel to and from home visits during shift. Call schedule
with weekend rotation.
