Family Practice-Geriatrics opening in Yakima, Washington. Seeking BE/BC Geriatric Medicine Physician to join our team in Yakima, WA. Provides primary care medical services to our senior care clients at participating assisted living, adult family homes and skilled nursing facilities. Some call required.Our goals: Improve the experience of care for clients

• Timely response to patients suffering and family concerns
• Coordination of all providers and their information
• Patients and family-centered decision making and care, respectful of patient autonomy and self-determination Improve care coordination
  • Peer to peer direct communication for care transitions
  • Electronic information exchange between the program and providers, hospitals and other facilities Clinic is the medical home for more than 30,000 people in Central Washington.
