Type: Travel Contract Shift Type: Monday-Friday, 8:00 AM - 4:30 PM
(Day) with rotating weekend coverage shared among five clinicians
(examples: work Saturday & Sunday with Thursday off and the
following Friday off); day shift with rotating weekends; not to
exceed 40 hours/week Hours per Week: 40 Candidate Pay Rate: $70 per

Responsibilities Provide compassionate in-home nursing care to Home
Health patients in private homes, assisted living, or adult foster
homes Perform thorough assessments, develop and update
individualized care plans, and document patient encounters
accurately Administer medications, treatments, and wound care per
physician orders and agency protocols Coordinate care and
communicate effectively with physicians, therapists, caregivers,
and the interdisciplinary team Educate patients and families on
disease management, medication adherence, and safe home care
practices Manage caseload independently with strong clinical
judgment and time management Drive to patient homes safely and
maintain professional representation of the agency in the community
Qualifications Active Registered Nurse (RN) license appropriate for
practice in Oregon BLS certification required Three or more years
of nursing experience (minimum) Home Health experience preferred
Reliable automobile and valid driver’s license (required for
in-home visits) Comfortable working independently with minimal
supervision and direction Strong interpersonal and documentation
skills Education Nursing diploma or degree (per state RN
requirements); Active RN licensure required Knowledge & Skills
Patient assessment and care planning in the home setting Skilled
wound care, medication administration, and clinical documentation
Ability to prioritize and manage a caseload autonomously Effective
patient and caregiver education techniques Familiarity with home
safety evaluation and infection control practices Good navigation
