to find a qualified Home Health RN in Hermiston, Oregon, 97838! Job
Title: Home Health RN Profession: RN Specialty: Home Health
Duration: 13 weeks Shift: Day Hours per Shift: 8 Experience: One
year of continuous RN experience in an acute care setting;
preferred previous home health or community health experience.
License: Current RN license in the State of Oregon; valid Oregon
Driver's License and proof of current auto insurance.
Certifications: BLS required Must-Have: Must have own automobile
for work. Description: Assesses, delegates, coordinates, and
evaluates the nursing care provided. Ensures the provision of
quality care for selected groups of patients through utilization of
nursing process, established standards of care, policies, and
procedures. Initiates the Plan of Care and necessary revisions.
Responsible for the overall care of the patient. Initiates
appropriate measures to prevent disease exacerbation, promotes
safety, and makes referrals to rehabilitative services. Coordinates
all services provided by other disciplines or organizations.
Provides essential medical care and support to patients in their
home setting, helping them to recover or manage chronic conditions
and related symptoms. Conducts ongoing assessments of patients'
physical needs and related symptoms. Develops an individualized
care plan based on the patients' needs and goals of care.
Communicates clearly and collaborates with patients, families,
caregivers, and providers. Communicates observations, facts, and
treatment suggestions as an interdisciplinary team member, both
individually and through team meetings. Ensures coordination of the
plan of care. Provides education to patients, families, and
caregivers on disease and symptom management. Maintains timely,
accurate, and detailed records of patient assessments, care plans,
treatment, and intervention responses/outcomes. Advocates for
patients and provides them support. Supports the organization's
mission, vision, values, policies, and procedures. Participates in
required education as applicable to position. Performs other
