Job BriefWISe Care Coordinator Yakima, WAEmployment duration:Full timeExempt Status:Non-exemptOffer Relocation?:NoA formal member of the Wraparound with Intensive Services (WISe) team who is specially trained to coordinate and facilitate the WISe process for an individual family and provide advanced care coordination activities within the phases and activities of WISe. The Care Coordinator is typically the facilitator of the CFT, and ultimately responsible for leading the team through the phases and activities of WISe both during and outside of the meetings. The Care Coordinator contributes knowledge and skills related to making sure that the team process honors each member's role, responsibility and perspective. The Care Coordinator is qualified and supported by completing the WISe training, participating in technical assistance, and is involved in ongoing WISe training and coaching activities.Qualifications:* Education: Bachelor's Degree in Social Work, Psychology, Counseling or other relevant field required.* Experience: Minimum of one year of experience working with families and children in a therapeutic or educational capacity.* Licenses/Certificates/Registration: Submit application for registration as an Agency Affiliated Counselor in the State of Washington within one week of hire. Current Washington State driver's license and proof of automobile liability insurance coverage; current First Aid/CPR card, or obtained within 45 days of hire; and a Children's Administration background clearance if working within the Placement Program. NOTE: FBI fingerprints required if you have lived in Washington State less than the last 3 consecutive years.* Strongly Preferred: Spanish/English bilingual proficiency.About YVFWCWe serve more than 181,000 patients across 25 medical clinics, 14 dental clinics, 10 pharmacies, and 64 program sites in two states.
