Our solutions include at-home clinical pharmacy support and an easier, safer way to manage medications.Job Details:Special Note on COVID-19 Measures : Many of the patients we serve are some of the most vulnerable to COVID-19. This includes the elderly and individuals with underlying chronic health conditions. At this time, it's important for these individuals to limit their contact with others.As a result of these critical health needs, and with respect to the health and safety of our employees, we have made a decision to conduct all assessments by phone until further notice. This is an important note as we are actively hiring several Clinical Liaisons throughout the United States but will be temporarily replacing in-home visits with phone assessments.Clinical Liaisons at ExactCare can be Certified Pharmacy Technicians (CPhT), healthcare licensed (LPN) or skilled professionals (case managers, social workers) that demonstrate excellent communication and relatability skills. All bilingual Clinical Liaisons will be responsible for clearly describing ExactCare's services and transitioning the patient from a local pharmacy to their new primary pharmacy, ExactCare. The bilingual Clinical Liaison is responsible for introducing ExactCare's innovative approach to helping patients manage medications in their homes throughout the Yakima, WA markets. This position is a vital piece of the ExactCare patient experience as our Clinical Liaisons are the face of our pharmacy-at-home services.The bilingual Clinical Liaison will conduct in-home assessments and educate prospective patients on the advantages of ExactCare's Chronic Care Pharmacy-at-Home--- model. This health care professional will build trust with patients by conducting in-home or telephonic assessments that will include a thorough medication management review. The goal of each Clinical Liaison is to educate and provide a detailed understanding of our medication adherence program and assist in enrollment services. This position is a fully remote, self-directed, traveling position. ExactCare will provide the bilingual Clinical Liaison with the necessary resources to provide an exceptional in-home patient experience.The ideal candidate will be a Certified Pharmacy Technician and have experience working with patients with complex, multi-medication needs. They will also feel comfortable conducting in-home and/or telephonic assessments related to medication management and be able to conduct daily travel.Essential Duties & Responsibilities:* Partner with the Clinical Liaison Field Manager in assigned territory to educate potential patients about ExactCare's medication management solutions.* Keep lines of communication open with the Clinical Liaison Field Manager to address any potential issues that may arise.* Collaborate with the ExactCare scheduling team regarding daily schedule, field travel and adjustments. Schedule patient for assessments during down when not completing assessments.* Manage the patient experience through medication assessments, collection of basic patient information and education on ExactCare solutions.* Conduct assessments with new patients each day by daily field travel throughout the assigned travel region or managing work queues for additional secondary work* Responsible for accurate and real-time data entry in compiling all patient medication data with the appropriate prescription information to ensure the patient will receive the exact medication at the exact time.* Maintain accurate records and prepare reports for the consistency of information to be shared with the Exact Care team.* Utilization of technology in the field to ensure timely and accurate capture of valuable patient data.* Demonstrate adaptability and flex to other patient care related tasks like making doctor calls for new and existing patient prescription requests.* Ability to meet or exceed key performance indicators that align with ExactCare's Patient Promise and organizational needs.* Other duties as assigned.Qualifications:* Current and unrestricted licensure as a Certified Pharmacy Technician (CPhT) or willingness to work towards a CPhT licensure within one year of hire.* Bilingual in Spanish/English is required* Current and unrestricted license as a Licensed Practical Nurse (LPN) will be highly considered* Degree in social work or at least six months social work, case management experience will be highly considered* Valid driver's license and active insurance* Experience with complex specialty pharmacy patients* Experience visiting members in their private residence, preferred* Medical social work or specialty pharmacy experience will be considered* Ability to establish trust with conviction and persuasion* Ability to partake in daily field travel throughout assigned region* Ability to travel nationally, a plus* Current/Past Use of technology to capture assessment electronicallyExactCare provides equal employment opportunity to all qualified applicants regardless of race, color, religion, national origin, sex, sexual orientation, gender identity, age, disability, genetic information, or veteran status, or other legally protected classification in the state in which a person is seeking employment.
