Direct Support Professional Supervisors: CEO/Administrator and C.O.O.; Director of Client Support and Director of Operations; Site Manager Definition: Responsible for overall support and life skills development for children and adults with developmental disabilities. Specifications: Prefer degree in social services, education, or related field. Preferably, one-year experience working with persons with disabilities, blood borne pathogens training, First Aid / CPR certification, Food Handlers Permit, TB test and positive crisis management training required. Duties: 1. Responsible for working cooperatively as a team member to meet the mission, philosophy and goal of the program. Responsible for assisting, teaching and supporting clients in the tasks of daily living.Responsible for assisting clients to plan for and access community integration activities.Responsible for assisting and supporting clients to arrange their daily schedules as needed.Responsible for fully implementing clients Individual Support Plans/Person Centered Support Plans, Instructional Individual Support Plans, and Positive Behavior Support Plan (if applicable).Responsible for tracking and keeping data regarding client's behavior and progress.Responsible for log notations to facilitate communication between shifts.Responsible for assisting and teaching clients personal hygiene routines, meal preparation, and other independent living skills as determined.Responsible for required record keeping.Responsible for participating in the development of Habilitative Goals and Plan of Care for clients as required.Responsible for assuring the safety and well-being of all clients including proper administration and documentation of medications, and teaching safety in the home and community.Responsible for maintaining confidentiality of clients regarding disclosure of information.Responsible for informing management of all concerns regarding clients as soon as possible or immediately if there is a grave concern.Responsible for working cooperatively and collaboratively with all support team members including Social Workers, Behavior Specialist, Case Managers, Educators, Guardians and family membersResponsible for working cooperatively and collaboratively with other programs providing services to the client to ensure the best possible experience and outcome for the client.Responsible for attending all scheduled staff meetings and staff training. This is mandatory.17. Responsible for performing additional related duties as assigned by management.
